General
Integrated Specialist Public Health Nursing ( ISPHN) Service
Description
Integrated Specialist Public Health Nursing -9 months to 19 (25) (ISPHNS)

The ISPHN service provides the Healthy Child Programme, Safeguarding and Public Health from the 28th week of pregnancy until your child leaves school and for those children with Special Needs the service continues until 25 years, mainstream and specialist education provision.  

The service deliver mandated contacts for under 5’s, as well as Early support and assessment and referral to the Early Support Panel for children with additional needs; this also involves ongoing signposting to other services and support as required by individual families, children and young people.

The ISPHN Service contributes to the development of Education Health Care Plans (EHCP) and establish partnership and collaborative working systems with families and other agencies to support families.

The Integrated Specialist Public Health Nursing Services are located throughout the East Riding area. Teams are based in:

•    Goole
•    West Wolds
•    Bridlington
•    North Holderness
•    South Holderness
The service is delivered in a range of settings Health Centres, Schools, Children Centres; community settings and the family home.



Access to this service
The Integrated Specialist Public Health Nursing service has contact with families in venues which are convenient and accessible to the family  eg home, Schools, children’s centres and Special schools.


Who this service is for
The Integrated Specialist Public Health Nursing Service is a universal service for all families, children and young people resident within the East Riding Local Authority area and those who attend schools within the East Riding area. 
 
By being resident in the area or attending school in the area you have access to the Integrated Specialist Public Health Nursing Service.  Families can self refer back into the service outside of the core programme and young people in secondary school can access t
How we communicate with children, young people with SEND and their families
Staff are not trained in Sign language but if required the interpretation services can be accessed.


Registered service and regulatory body

We are an NHS service regulated by the Care Quality Commission. The Nursing and Midwifery Council is the regulatory body for individual qualified nurses within the services.

Training acquired to support children and young people with SEND and their families
Regular training is being provided by the East Riding CCG Special Educational Needs and Disabilities (SEND) Designated Clinical officer.
