Futures +
Description
We provide a range of services which look to promote social inclusion and independence for young people and adults with a learning disability / autism / Asperger’s and help young people to think about their futures through the transition from childhood to adulthood.

Some examples of work undertaken include: person centered planning, attending  person centered Education, Health and Care Plan meetings in preparation for adulthood, independent travel training, travel pass assessments, life skills .

There are also specialist social workers based within the team who can assess for an adult (transition) assessment in preparation for adult life. Social workers will work with existing social care staff to ensure that the transition from children’s services to adult services is a positive experience for all involved.

We will meet up with people at their own homes or at an alternative venue dependent upon the service requested.

Referrals can be made by contacting Customer Services on (01482) 393939 or calling into a Customer Service Centre.
Other professionals involved in the young person’s support can also make a referral. 
Who this service is for
The service is for young people and adults with a  learning disability / autism or aspergers.
Parent and sibling involvement
Parents do not need to stay while the child / young person accesses the service.

Training acquired to support children and young people with SEND and their families
All staff have learning disability training.
Staff have also accessed ERSCB Training – Awareness of Child Abuse, Safeguarding Children with Disabilities, Safeguarding Children from Neglect, Domestic Violence Awareness.
Opening times
Below are the office hours, however support times can be flexible to commence earlier and finish later than the office hours – for example – start at 7.30am and finish later in the evening (up until 10pm). We can also carry out weekend working when required.
